Need to customize the sign up form

Hello.

I'm looking for someone who can connect with our font end developer and help him customize a form (in a css exit pop up window) on some of our web assets that would just have an email field and a subscribe button. After a visitor enters their email address and clicks submit a new tab should open with an external link. The external links have to be modified on our web assets directly and not in the MailWizz app as we have to change them often and there are other scripts that dynamically add variables to those external links.

Please let me know if you can help. I would like you to also explain the developer how you have done it so they can do it next time themselves.
