List form custom redirect/webhooks segmentation

Hi,
Has anyone experience or knowledge to redirect "list page using extension list form custom redirect" based on segmentation instead of global for a list ?

for example, every "subscription confirmed page":
Segment | Redirect to
----------------------------------
A | http://a.com/urlA
B | http://a.com/urlB

That's not possible easily.
We have a hook there, so with programming one can do it.

I can see the method type there is POST and GET, How to send the data into POST ?

This sends data to your url of choice, and there you have to process that data, which comes in GET or POST.
