How to build a form in Mailwizz

I can't find any documentation showing how to build a form in MailWizz.
I want to embed this form on my website so that people who complete the form are put on an auto-responder sequence.

Would love any guidance in figuring out how to do this.

How do I change what the embed form is. For e.g I don't want the last name, Just the first name and email?

Either this. Or you cane dit the list custom fields and remove the fields you don't need, or hide them from there.
