Duplicate contacts

Hi all! We've hit the duplicate contacts issue. The segment we used initially showed only xx subscribers, but once the campaign was sent out, we ended up delivering to xxxx, and we got about 2,394 duplicates.

What might be the issue causing the contacts to be duplicated? Has anyone experienced the same issue?
