How can I use 1 form while using multiple thank you pages

Hi guys
I'm trying to accomplish the following:

my assets are:
OptimizePress WP plugin
Thrive Leads WP plugin
Mailwizz app

I want to have different free checklist for every blog post I publish BUT want all the leads in 1 LIST

so, I want to use the same form html but with multiple different thank you / download page for each checklist as it will be different for each blog post.

Anyway I can accomplish this?

An integration tool helps as an intermediate step, so you take leads from subscription forms into the integration tool, and then by api into mwz. The thank you pages are not served by mwz anymore, so you are not bound by one per list.
