When I try to open my [SUBSCRIBE_LINK], I see the following form. How could I add or remove fields here?

In your list on Custom Fields and set Visibility to none or hidden and then these fields will not be visible in your form.

But i have a lots of fields whose visibility is "Visible", it seems that they are not displayed in the subscribe form, why?

Can you check if you are on the correct list subscribe form? If these custom fields exist in your list then these need to appear on subscribe form.
